This office is currently affiliated with PMD, Viva, United Healthcare, and Aetna. If you are a member, inform us upon checking in and present your identification card to the receptionist so that we may verify your coverage. Co-pays are due on the day of your visit.

As participating providers in the Medicare program, we file your claims. You are responsible for the full deductible. If you have met your deductible, we will be reimbursed 80 percent of the Medicare allowable charge. Therefore, when services are rendered, you are asked to pay 20 percent of the allowable charge. If you have supplemental insurance for the 20 percent, we will file for you.

Assignment on Champus claims is not accepted and payment is expected in full at the time of service. However claims will be filed for Champus patients, so that Champus can reimburse the patient.

Office visits are by appointment. Please inform us 24 hours in advance if you cannot keep your appointment, so that time can be given to another patient. Occasionally it may be necessary to reschedule a visit, and every attempt will be made to do so at your convenience. Due to a full schedule, it is not possible to see drop-in patients. Return appointments normally will be made as you are leaving the office. If you have any questions regarding scheduling, do not hesitate to ask the receptionist at 334-396-1555.  There will be a $30.00 fee for missed appointments unless cancelled 24 hours in advance.  If the appointment is surgical or procedural, the fee is $100.00.

The physician may write a prescription for your condition and it will indicate appropriate refills. Please phone your pharmacist for refills, or you may phone us during regular office hours. Some medications require follow-up visits in order to be refilled.

During office hours, the physician or his nursing assistant will return your call as soon as possible. During non-office hours, the physician may be reached through the Baptist Hospital switchboard.
